At first I assumed the failure of this parachute to deploy was just dumb bad luck. This was one of four parachutes in the tube.  The other three were redundant and connected to the video & altimeter payload, and this last one was for the booster.  The payload ejected and took the other three parachutes with it, but there really was nothing to pull out this fourth parachute - the ejection gasses just blew around this loosely packed chute.  Result was this bad body tube damage.  Next time I'll pack the other chutes first and put this booster parachute on top, so when the other chutes are pulled out they will drag this one out with them.
